Select (Request a Pharmacist Assessment) on the homepage and complete the existing pharmacy application form.
4. Does submitting the form guarantee treatment or supply?
No. The pharmacy team will review your application and advise you of the next steps.
5. How do I access my patient account?
Use (Patient Sign In) in the site navigation. Access to restricted pharmacy information depends on the pharmacy’s existing authorisation process.
